The problems WhatsApp solves for clinics and outpatient practices

  • The front desk phone never stops

Receptionists answer the same questions all day: is the doctor in today, what are the timings, how much is the consultation, where do I park. Patients who cannot get through on the phone walk into another clinic nearby.

  • No-shows leave gaps in the schedule

A booked slot that nobody turns up for is time the doctor cannot sell again. Patients rarely cancel; they simply forget or decide not to come, and the clinic finds out when the slot is already lost.

  • Follow-ups and recalls slip

A dental patient due for a six-month cleaning, a physiotherapy patient who stopped after three sessions or a diabetic patient due for a review are revenue and care that quietly disappear when nobody reminds them.

  • Doctors' personal numbers are overused

Patients who once got the doctor's mobile number send reports and questions at all hours. The doctor has no help, no records and no boundary.

Why clinics move booking to WhatsApp

The whatsapp api for clinics does three jobs well: it lets patients book a real free slot in a chat at any hour, it reminds them the day before with one-tap confirm or reschedule, and it brings them back when a prescribed follow-up falls due. The front desk keeps control through a shared inbox, and the doctor's personal phone stops being the clinic's helpline.

This page is for outpatient practices: general physicians, dental, eye, dermatology, physiotherapy and diagnostic collection centres. Multi-specialty hospitals have more departments and more privacy layers; see the hospital playbook. If you are new to the platform, start with what the WhatsApp Business API is.

Message templates to get approved first

Clinic messages that you start must use an approved message template. Sample values are in brackets. None of these mention a diagnosis.

Template nameCategorySample
appointment_bookedUtilityHi {{1}}, your appointment with {{2}} is booked for {{3}} at {{4}}. Reply here if you need to change it. (Meera; Dr Kavitha Rao; Tue 21 Oct; 5:30 pm)
appointment_reminder_day_beforeUtilityHi {{1}}, a reminder of your appointment with {{2}} tomorrow at {{3}}. Tap Confirm or Reschedule. (Meera; Dr Kavitha Rao; 5:30 pm)
slot_cancelled_rebookUtilityHi {{1}}, your appointment on {{2}} has been cancelled as requested. Reply BOOK to choose a new time. (Rahul; Thu 23 Oct)
followup_dueUtilityHi {{1}}, {{2}} had advised a follow-up visit around {{3}}. Would you like to book a slot? (Priya; Dr Kavitha Rao; late October)
report_ready_collectUtilityHi {{1}}, your reports from {{2}} are ready. You can collect them at the front desk or ask us to share them here. (Sanjay; 18 Oct)
health_camp_inviteMarketingHi {{1}}, {{2}} is holding a free check-up camp on {{3}}. Reply CAMP to book a slot. (Anil; Sunrise Dental Clinic; 9 Nov)

Note that report_ready_collect says the report is ready, not what it says. Share the report itself only inside the chat, after the patient asks for it.

Booking chatbot flow outline

  1. Trigger: New Message, plus Match Keyword for BOOK, RESCHEDULE and CANCEL; Click to WhatsApp Ads if you run local ads.
  2. Send Message: greeting with the clinic name and a three-button service choice.
  3. Ask Question: patient name, and for returning patients the registered phone or patient ID (validated as a number).
  4. Native Calendar (Add Automation App): fetch free slots for the chosen doctor, show them as a list of up to 10, and create the booking.
  5. Save Variable Value: store the booking time and doctor on the contact, so the reminder template can use them.
  6. AI Intent Detector: if the patient types symptoms or an emergency, stop the bot and Assign to Agent, with an instruction to call the clinic or emergency services.
  7. Working Hours: outside hours, explain when the desk will reply.
  8. Smart Wait: for recalls, wait until the follow-up date saved after the last visit, then send followup_due.

Broadchat features to use

  • Automations → Calendar for doctors' event types, weekly availability, slot interval, patients per slot and the bookings list.
  • Chatbots → Chatbots for the booking flow; Chatbots → Analytics to see where patients drop off.
  • CRM → Super Field for patient ID, doctor, last visit and next-due date; CRM → Tags for treatment type.
  • CRM → Live Chat for the front desk, with CRM → Canned Replies for directions, parking and fees.
  • Workspace → Roles & Permissions so only clinical staff open chats tagged for reports.
  • AI → AI Agents with instructions limited to administrative FAQs.
  • Payments → Payment Gateways if you take advance booking fees through Razorpay or PayU.

Illustrative ROI

An illustrative calculation, not a promise: a dental clinic books 400 appointments a month and 48 patients do not turn up. If day-before reminders with a reschedule button recover 16 of those slots, either by the patient attending or by the slot being rebooked, and the average visit is worth ₹1,200, that is ₹19,200 a month. Recalls add more if even a few patients come back for follow-up treatment. From this, subtract Meta's charges for the reminder templates and the platform subscription; see WhatsApp API pricing explained. Count your no-shows for a month before you start.

Getting the details right

Send reminders at a sensible hour, usually the previous afternoon. Do not send several reminders for one visit; one reminder with clear buttons works better and protects your quality rating. For the reminder flow in more detail, see the appointment reminders use case. Healthcare marketing agencies can set this up for many clinics under their own brand through the partner program.

Use cases with example messages

  • Appointment booking chatbot

Patients pick a doctor or service, then choose from real free slots on a calendar that knows each doctor's weekly availability and how many patients fit in a slot. The booking is confirmed instantly and the front desk sees it in the bookings list.

    Reply inside the 24-hour window

    1. Business: Welcome to Sunrise Dental Clinic, Indiranagar. What would you like to book? [Check-up and cleaning] [Tooth pain] [Braces consultation]

    Example message
  • Appointment confirmation and reminder

Send a utility confirmation as soon as the slot is booked and a reminder the day before, with Confirm and Reschedule buttons. Rescheduling requests go to the front desk; cancellations free the slot for someone else.

    utility template

    1. Business: Hi Meera, this is a reminder of your appointment with Dr Kavitha Rao at Sunrise Dental Clinic tomorrow, Tue 21 Oct, at 5:30 pm. Please arrive 10 minutes early. [Confirm] [Reschedule]

    Example message
  • Post-visit instructions

After a procedure, send the care instructions the doctor approved, such as what to eat after an extraction, as a PDF or short text, plus the date of the next visit.

    utility template

    1. Business: Hi Arjun, thank you for visiting Sunrise Dental Clinic today. Your post-extraction care sheet is attached. Your stitch-removal visit is booked for Sat 25 Oct at 11:00 am.

    Example message
  • Recall and follow-up reminders

Remind patients when a follow-up the doctor prescribed falls due, such as a six-month cleaning or a three-month review. Keep the message about their care plan, without promotional offers, so it stays a utility message.

    utility template

    1. Business: Hi Priya, Dr Kavitha Rao had advised a follow-up check-up around this time after your root canal treatment in April. Would you like to book a slot? [Book now] [Remind me next month]

    Example message
  • Health camp and seasonal check-up announcements

Announce a free dental camp, a new service or a seasonal check-up package to patients who agreed to receive updates. This is marketing and needs separate consent.

    marketing template

    1. Business: Sunrise Dental Clinic is holding a free oral check-up camp for children on Sunday 9 Nov, 10 am to 2 pm. Book a slot for your child by replying CAMP. Reply STOP to stop updates.

    Example message
  • Front-desk questions answered by an AI agent

An AI agent whose instructions contain the clinic's own FAQ answers timings, fees, parking and insurance questions, and hands anything clinical to the staff. It never gives medical advice.

    Reply inside the 24-hour window

A rollout playbook

  1. Set up the calendar first

Create an event type for each doctor or service with duration, weekly availability and patients per slot. Every other step depends on knowing which slots are really free.

  2. Collect consent at registration

Add one line to the registration form for appointment and care messages on WhatsApp, and a separate, optional tick for health tips and camp announcements.

  3. Get reminder templates approved

Submit the confirmation, day-before reminder and follow-up recall templates first. Keep them free of offers and free of diagnoses.

  4. Launch the booking chatbot

Start with three or four services and a clear Talk to front desk option. Test it on staff phones before you put the link on Google and your website.

  5. Add recalls by treatment

Save the next-due date in a custom field after each visit, then send recall messages from a scheduled flow so no follow-up depends on someone's memory.

  6. Keep the doctor's personal number private

Move patient queries to the clinic number and let the front desk triage. Doctors can answer from the shared inbox on the mobile app when needed.

Compliance notes

  • Health data is sensitive personal data. Under India's Digital Personal Data Protection Act, 2023, collect clear consent for a stated purpose and use the data only for that purpose. Take legal advice for your clinic.
  • Never put a diagnosis, test result or medicine name in a template without the patient's explicit agreement to receive it on WhatsApp; a phone may be seen by family members.
  • Clinical advice must come from a registered practitioner. Configure chatbots and AI agents to answer only administrative questions and to hand over anything clinical.
  • Restrict who can open patient chats using roles, and do not export chat histories to personal devices or spreadsheets shared outside the clinic.
  • Appointment reminders and recalls are not consent to marketing. Ask separately before sending camp announcements or package offers.
  • Avoid claims such as "painless", "guaranteed cure" or before-and-after promises in campaigns; medical advertising in India is regulated and Meta may reject such templates.

Frequently asked questions

Is it safe to send medical reports on WhatsApp?

WhatsApp messages are end-to-end encrypted in transit, but the phone can be seen by others and the business side is stored in your platform. Send reports only when the patient asks for them in the chat, and limit which staff can open those chats.

Are appointment reminders marketing messages?

No. Reminders and confirmations for a booking the patient made are utility messages. They become marketing if you add offers or promote other services.

Can patients book without calling the clinic?

Yes. A chatbot connected to the native calendar shows only free slots for the chosen doctor and books one, and the front desk sees it in the bookings list immediately.

Can the chatbot answer medical questions?

It should not. Configure it for timings, fees, directions and booking, and hand any symptom or clinical question to staff. Clinical advice must come from a registered practitioner.

Can we keep our clinic number on the WhatsApp Business app?

Yes. In India, coexistence lets the number stay active on the WhatsApp Business app while it is connected to the API, so staff can still reply from the phone. Some app features are switched off.
